Public health plays an important, albeit often unnoticed, role in protecting and promoting the health of populations. The activities of public health are complex, performed by multiple professionals, and range from the innocuous to the intrusive. Ethical analyses in public health reflect some of this complexity and fragmentation, with no one approach able to capture the full range of ethical considerations raised by public health activities. There are however, good reasons why we should pursue such analyses. Providing a robust ethical (...) framework for public health may promote the identity and function of public health, address some of the shortcomings of utilitarianism, and help to combat the threat that public health faces through lack of political will in many parts of the world.In this paper I argue that Alasdair MacIntyre’s account of practices and virtues can make a valuable contribution to public health ethics. The first part of the paper argues that public health may properly be described as the type of practice that provides an arena for the exercise of virtues. This is followed by an analysis of the three virtues of honesty, courage and justice in public health practice. Using virtue theory captures morally important elements of public health and helps to maintain awareness of significant moral values in the practice of public health. Such awareness is crucial in maintaining and defending the integrity of public health. (shrink)

Les principales altérations rencontrées sur les vestiges en marbre et en granite de Délos sont décrites et les mécanismes physico-chimiques de leur développement sont expliqués en tenant compte des processus environnementaux qui conditionnent leur formation. Deux facteurs principaux d'altération ont ainsi été mis en évidence : les sels marins et la pollution atmosphérique. Impliqués dans les cycles de dissolution-cristallisation, ces sels forment des desquamations, des désagrégations et des cratères. Ces cycles résultent de l'évaporation des solutions migrant depuis le sol par (...) capillarité et de la solvatation des sels déposés sur les pierres par les embruns. More than 200 million children under the age of 5 years are affected by malnutrition worldwide according to the World Health Organization. The Barbados Nutrition Study (BNS) is a 55-year longitudinal study on a Barbadian cohort with histories of moderate to severe protein-energy malnutrition (PEM) limited to the first year of life and a healthy comparison group. Using quantitative electroencephalography (EEG), differences in brain function duringchildhood(lower alpha1 activity and higher theta, alpha2 and beta activity) have previously been highlighted between participants (...) who suffered from early PEM and controls. In order to determine whether similar differences persisted intoadulthood, our current study used recordings obtained during a Go-No-Go task in a subsample of the original BNS cohort [population size (N) = 53] at ages 45–51 years. We found that previously malnourished adults [sample size (n) = 24] had a higher rate of omission errors on the task relative to controls (n= 29). Evoked-Related Potentials (ERP) were significantly different in participants with histories of early PEM, who presented with lower N2 amplitudes. These findings are typically associated with impaired conflict monitoring and/or attention deficits and may therefore be linked to the attentional and executive function deficits that have been previously reported in this cohort in childhood and again in middle adulthood. The different situations encountered by a professional make up the frame of the analysis of the working activity. These situations are analyzed as learning potentiality and as development of the workers and learners . We hypothesize that different situations are to be mobilized to rethink a personal and professional route whose finality is learning and development of the worker . These situations have to be thinking like a set of opportunities to learn and to grow, in other words, as carriers (...) of formative experiences situations. These encounter of situations could taking sens by a dialogical process or dialectic established between different situations. To support this thesis, we propose to identify and characterize these carriers learning and development situations. To do this, we propose to appeal to the experiential posture of these different situations , especially some of them , which are considered to be representative and often iconic vocational training , that is to say training period. Les différentes situations rencontrées par un professionnel constituent le cadre d’analyse de l’activité du travail. Ces situations sont à analyser comme des potentialités d’apprentissage et de développement d’un sujet travailleur et apprenant. Nous faisons l’hypothèse que différentes situations sont à mobiliser pour penser autrement un parcours personnel et professionnel dont la finalité serait l’apprentissage et le développement du sujet. Les situations seraient ainsi pensées comme un ensemble d’occasions d’apprendre et de se développer, autrement dit, comme des situations porteuses d’expériences formatrices. Cette rencontre de situations prendrait sens selon un processus dialogique voire dialectique institué entre les différentes situations. Pour soutenir cette thèse, nous proposons d’identifier et de caractériser ces situations porteuses d’apprentissage et de développement. Pour ce faire, nous proposons de convoquer la posture expérientielle de ces différentes situations, en particulier certaines d’entre elles, dont on considère qu’elles sont représentatives et souvent emblématiques des formations professionnelles, c’est-à-dire les stages. Despite the increasing focus on rationing, and rationing decisions in the NHS, little attention has been given to patient's perceptions of rationing and the potential impact this might have on people's use of services. Drawing on the qualitative findings of a study conducted in the North West of England which was concerned with the pattern and processes of primary care help seeking, this paper sets out to examine perceptions and experiences of rationing in primary care and the potential impact this (...) has on people's use of services. In relation to primary care services people had experienced rationing by deterrence, dilution and delay. There was some evidence that perceptions of rationing impacted on help seeking and the use of primary care services. The implications for understanding the way in which perceptions of rationing might influence the formulation of demand and help seeking by people using primary care services are discussed. (shrink)

This study examined the laboratory dream content reported by 14 patients with schizophrenia and 15 controls, with a focus on reports obtained from NonREM sleep. Both the controls’ and patients’ frequency of dream recall following awakenings from NonREM and REM sleep were similar to values reported for healthy participants. Patients’ NonREM sleep narratives were shorter than those from controls. When compared to their reports from REM sleep, both groups’ NonREM sleep reports included significantly fewer words and reportable items. The controls (...) were more likely to report a subjective feeling of bizarreness for their REM sleep reports as compared to their NonREM sleep reports. This difference was not observed in patients with schizophrenia. Taken together, these findings suggest few differences between the NonREM sleep mentation of patients with schizophrenia and of controls and that sleep stage cognitive style is comparable in both groups, with NonREM sleep reports being more thought-like, less elaborate and bizarre than REM sleep reports. The mass childhood immunization programme has traditionally been viewed as a safe and effective preventative measure by health promoters, primary health care professionals and governments. This consensus has meant that immunization has rarely been viewed as ethically problematic. A number of recent changes in the context of the delivery of health care, particularly the emphasis on consumerism and the effect of the marketization of services, makes timely an examination of ethical, social and political issues. This article examines four main grounds (...) for problematizing the mass childhood immunization programme. These are: clinical research evidence about the safety and efficacy of vaccines; the masking of wider social and political determinants of ill health; the contradictory strictures about collective and individual rights in relation to immunization; and the uniqueness of childhood immunization as a physical intrusion into a healthy body. The implications of these ethical issues are discussed in relation to informed consent and the need for a 'greenfield' review that includes the views of dissenting parents, lawyers and moral philosophers, as well as health professionals. Les promoteurs de santé, les soignants et les gouvernements ont tous traditionellement vu le programme d'immunisation globale des enfants comme sain et efficace. Cela voulait dire que l'immunisation a rarement posé un problème éthique. Pourtant, actuellement des changements dans les contextes de la distribution des soins, le consumérisme et de l'effet de la commercialisation des services de santé, exigent une revue des problèmes éthiques, sociaux et politiques. Cet article discute quatre éléments problématiques du programme d'immunisation globale: les données des recherches médicales sur la sûreté et l'efficacité des vaccins; la dissimulation des déterminatifs sociaux et politiques des maladies; les critiques contradictoires des droits individuels et collectifs concernant l'immunisation; et le caractère unique de l'immunisation des enfants comme une intrusion physique dans un corps sain. Les effets de ces questions éthiques sont repris en rapport du consentement informé et de la nécessité d'une revue 'verte' qui comprend les vues des parents dissidents, des juristes, des philsophes et des membres des professions soignantes. Das Massenimpfungsprogramm von Kindern wurde allgemein von Gesundheits behörden, medizinischem Fachpersonal und Regierungen als sichere und wirksame Vorbeugungsmassnahme betrachtet. Diese Uebereinstimmung bedeutete, dass Imp fungen selten ein ethisches Problem darstellten. Eine Anzahl von Aenderungen im Gesundheitswesen - besonders im Hinblickin auf Konsum und Marktwirtschaft im Dienstleistungsbereich - berechtigen jedoch zu einer Untersuchung der ethischen, sozialen und politischen Fragen. Dieser Artikel untersucht vier Hauptgründe, die das Massenimpfungsprogramm für Kinder in Frage stellen: Forschungsergebnisse über die Sicherheit und Wirksamkeit der Impfstoffe; die Verschleierung von andern sozialen und politischen Krankheitsursachen; die widersprüchlichen Anordnungen über kollektive und individuelle Rechte mit Bezug auf die Impfungen; und die aussergewöhnliche Art der Impfung als physische Verletzung eines gesunden Körpers. We agree with Faunce’s proposal that academic legitimacy is important in ensuring that whistle-blowing is included in medical curricula. We disagree, however, with the assertion that this is best achieved by means of an over-arching theoretical foundation for health care whistle-blowing of the kind suggested by Faunce. We propose that systematic theoretical justification is neither the sole nor the main determinant of academic legitimacy when it comes to matters for inclusion in medical school curricula, and outline an alternative view, together (...) with a practical example of a healthcare whistle-blowing topic. The theoretical and empirical link between leadership and workplace bullying needs further elaboration. The aim of the study is to examine the relationship between quality of leadership and the occurrence of workplace bullying 2 years later. Furthermore, we aim to examine a possible mechanism from leadership to bullying using social community at work as mediator. Using survey data that were collected at two different points in time among 1664 workers from 60 Danish workplaces, we examined the total, direct and indirect (...) effects between quality of leadership and workplace bullying. Our results indicate that quality of leadership plays a role in establishing working conditions that lead to workplace bullying. Furthermore, social community at work fully mediates the effect of poor quality of leadership on workplace bullying. This longitudinal study adds to previous cross-sectional studies on the substantial role played by leaders in the bullying process. Within the leadership–bullying relationship, social community at work acts as a full mediator, adding a significant contribution to the discussion of mechanisms involved in the bullying process. Plausible explanations of this mechanism and practical implications are discussed. (shrink)

This is the text of the Lindley Lecture for 2018 given by Roger Crisp, a Professor of Moral Philosophy at St. Anne’s College, Oxford.

In her book Michel de Montaigne: Accidental Philosopher, Ann Hartle argues that Montaigne’s thought is dialectical in the Hegelian sense. Unlike Hegel’s progressive dialectic, however, Montaigne’s thought is, according to Hartle, circular in that the reconciliation of opposed terms comes not in the form of a newly emergent term, but in a return to the first term, where the meaning of the first is transformed as a result of its dialectical interaction with the second. This analysis motivates Hartle’s claim that (...) Montaigne is not a skeptic at all, let alone a Pyrrhonian skeptic. In this paper, I argue that Hartle’s circular-dialectical interpretation of Montaigne is not only compatible with Pyrrhonism, but is in fact an ideal model for understanding Sextus Empiricus’s skeptical therapy. Over the past 20 years, there has been a shift in history education away from a view of history as the pursuit of an objective, universal story about the past toward ‘historical consciousness,’ which seeks to cultivate an understanding of the past as something that makes moral demands on us here and now. According to Roger Simon, historical consciousness calls us to ‘live historically’ – to live in a particular kind of ethical relationship with the past. However, no matter how (...) much educators might believe in the importance of learning to live historically, if students do not see it that way, even the most thoughtfully designed program in history education will never get off the ground. This paper is an attempt to see whether the notion of ‘caring for the past,’ based on Nel Noddings' ethic of care, might help us begin to address the problem of moral motivation in educating for historical consciousness. Les "grandes idées" identifiées sont rabâchées, ressassées, réduites, donnant lieu à des associations aussi rapides que funestes, telle celle qui fait d'Emmanuel Levinas "le philosophe de l'autre". L'enseignement universitaire du XIIIe siecle se fonde sur le commentaire et la dispute. La Sophistria de Robertus Anglicus (1260-70), dont est proposee ici l'edition critique accompagnee d'une etude historique et doctrinale detaillee, est un temoin exceptionnel de cette pratique de l'enseignement des arts par mode de sophisme, surtout developpe pour la logique et la grammaire. Il s'agit d'une collection d'une trentaine de sophismes, organisee de facon systematique, et preservee dans huit manuscrits. On y voit ainsi, en partant du sophisme, enonce (...) problematique, le maitre avancer des arguments, avec ou contre ses bacheliers, a propos de toutes les difficultes qu'il contient, les arguments et solutions fournissant au bout du compte un expose general de la syntaxe latine. La Sophistria s'insere dans une tradition de la grammaire speculative, qui, contrairement aux grands traites posterieurs sur les Modes de signifier, met l'accent sur la semanticite plus que sur la grammaticalite: un enonce incorrect peut etre admis si l'on comprend la raison (ratio) qui rend compte de sa deviation. Cette approche, comme l'application tres caracteristique de la Physique d'Aristote a la grammaire, permet de rapprocher la Sophistria de l'enseignement parisien des maitres anglais de la generation precedente, Robert Kilwardby et Roger Bacon, l'appartenance au milieu parisien se confirmant par sa proximite avec le Tractatus de Gosvin de Marbais. In this paper I explore the connection between narrative ethics and the increasing emphasis on historical consciousness as a way to cultivate moral responsibility in history education. I use Timothy Findley’s World War I novel, The Wars, as an example of how teachers might help students to see history neither simply as a collection of artefacts from the past, nor as an effort to construct an objective view about what went on in those other times and places, but rather as (...) something that makes ethical demands on us here and now. Theoretically, this paper draws on Adam Zachary Newton’s conception of narrative ethics and Roger Simon’s conception of historical consciousness, both of which rest on the Levinasian themes of irreducible difference, the face, and subjectivity as a position of ethical responsibility to and for the other. Nurses have moral obligations incurred by membership in the profession to participate knowingly in health policy advocacy. Many barriers have historically hindered nurses from realizing their potential to advance health policy. The contemporary political context sets additional challenges to policy work due to polarization and conflict. Nursing education can help nurses recognize their role in advancing health through political advocacy in a manner that is consistent with disciplinary knowledge and ethical responsibilities. In this paper, the authors describe an exemplar of (...) Elizabeth Barrett's “Power as Knowing Participation in Change” theory as a disciplinary lens within a doctoral nursing health policy course. Barrett (radically) emphasizes “power as freedom” instead of “power as control.” This approach is congruent with nursing disciplinary values and enhances awareness of personal freedom and building collaborative relationships in the policy process. The theory was used in concert with other traditional policy content and frameworks from nursing and other disciplines. We discuss the role of nursing ethics viewed as professional responsibility for policy action, an overview of Barrett's theory, and the design of the course. Four student reflections on how the course influenced their thinking about policy advocacy are included. While not specific to policymaking, Barrett's theory provides a disciplinary grounding to increase students' awareness of freedom and choices in political advocacy participation. Our experience suggests that Barrett's work can be fruitful for enhancing nurses' awareness of choices to participate in change across settings. (shrink)
